I was fortunate enough (and really happy) to get an answer from Valve regarding the current situation of custom sounds on add-ons. Here's some info. Per-map sound caches are not possible in L4D2, just a unified _master.cache. I'm sure a lot of you suspected or assumed that already. The fuzzy part of this topic was packing _master.cache into add-ons. It's been recommended that level designers do not do that.
